• For the security and confidentiality of our patients, medical records requests cannot be accepted via email. To ensure accurate processing and a timely response, all requests must be submitted by fax to (478) 240-2377.
• A valid photo ID is required to pick up medical records.
• Please note, one cannot pick up medical records for another person unless you have a note from that individual.
• In some cases, charges will apply.
