POSITION DESCRIPTION:
The Social Services Coordinator informs patients of their rights by distributing and explaining the “Statement of Patients’ Rights”, supports their interests if there is a violation of those principles and ensures patients and their families receive care and medical treatment in all Extended Care Facility departmental operations in accordance with WCRMC Extended Care Facility’s mission. The Coordinator participates and maintains performance improvement activities to assure highest standards of care as well as develops, organizes and implements statistical “customer satisfaction” evaluation tools and presents the findings to ECF task force committees for continuous improvement. The Coordinate ensures resident’s financial aspects are maintained and documented.
Performs other duties as assigned.
EDUCATION: Graduate of an accredited high school or GED program.
EXPERIENCE: At least three (3) years appropriate experience in a healthcare facility.
LICENSE: Completion of a Georgia Licensed Clinical Social Worker certification (or Georgia certification in related field) OR licensure qualification can be met with two years’ experience specifically in a Long Term Care (LTC) facility.
