MDS-Care Plan Coordinator: Registered Nurse (RN) /Licensed Practical Nurse (LPN)-ECF
Washington County Extended Care Facility is seeking a Registered Nurse (RN) /Licensed Practical Nurse (LPN) MDS-Care Plan Coordinator to join our team. This role is responsible for providing high-quality nursing care to our geriatric residents, coordinating care plans, and supervising day-to-day nursing activities in accordance with federal, state, and local guidelines.
Key Responsibilities:
- Provide and oversee direct nursing care for residents.
- Lead and supervise nursing assistants to ensure quality care.
- Complete MDS assessments and develop individualized care plans.
- Participate in resident assessments, care planning conferences, and ongoing evaluations.
- Ensure compliance with safety, infection control, and facility policies.
- Maintain accurate documentation, charting, and reports.
- Serve as a resource and leader to staff, residents, and families.
Qualifications:
- Graduate of an accredited RN or LPN program.
- Current, unencumbered RN or LPN license in the state of Georgia.
- Previous long-term care experience preferred.
- Strong leadership, assessment, and care planning skills.
- Ability to work effectively with residents, families, and staff.
