Washington County Regional Medical Center (WCRMC) has proudly provided patient-centered care to our community for over 60 years. Our hospital is a licensed acute care facility with 56 beds and a 60-bed Extended Care Facility.
We are seeking a compassionate and driven Case Manager to help guide patients through their healthcare journey and ensure they receive coordinated, high-quality care.
🩺 Key Responsibilities
- Assess patients’ medical, psychosocial, and financial needs
- Develop and manage individualized care plans
- Coordinate care across providers for seamless service
- Educate patients and families on treatment options and resources
- Assist with pre-authorizations and appeals
- Monitor progress and adjust care plans as needed
- Maintain accurate documentation and case tracking
- Identify barriers and connect patients with support services
- Advocate for patients during and after their stay
✅ Qualifications
- Graduate of an accredited Nursing School
- Current RN or LPN License (GA)
- Current BLS Certification
- Minimum 1 year Case Management experience
